EPS Nomination Form

The Electronic Prescription Service (EPS) allows your GP to send your prescriptions electronically to your chosen pharmacy, making it quicker and easier to collect your medication. By completing our EPS Nomination Form, you can nominate Pharmacy@Roxton as your preferred pharmacy for future prescriptions. This helps make the prescription process more convenient, saves time, and means your medicines can be prepared ready for collection once your prescription is issued.

    By ticking this box you are consenting to your future prescriptions being sent electronically to Pharmacy@Roxton and Pharmacy@Freeman Street. We will then dispense your prescriptions and can collect at the pharmacy. You can change this nomination at any time.
